The Illinois Department of Public Health has finally produced the Vaccine Registry opt-out form which is required to be provided at least once to each person receiving a vaccine in Illinois. To date, we have not heard from a single person who has ever been provided this required document. We first covered this matter in this article.
We can only wonder why it took IDPH thirteen days to provide a simple form required by law to be provided to every person receiving a vaccine. The meta-data in the form provided reflects it was created on November 8th, 2021, at 9:59 am. It was provided to us seventeen minutes later.
