Mandated Vaccine Registry Opt Out Form Not Being Provided

In yet another example of people not following the law, we once again have been unable to find a single vaccinated person who was provided with the mandated printed immunization data exemption form.  During a recent event this past weekend I asked a crowd of over 100 people, with many confirming they were vaccinated, if they received the required form and not a single person had ever heard of it, let alone got a printed copy as required by law.

Before immunization data is to be entered into the state registry, the immunization providers are required by law to provide a printed immunization data exemption at least once.

(410 ILCS 527/17) Sec. 17. Immunization data exemption form; immunization providers. Before entering immunization data into the immunization data registry, authorized immunization providers who provide immunization data into the immunization data registry shall provide the patient or the patient’s parent or guardian, if the patient is less than 18 years of age, with a printed immunization data exemption form at least once. The printed immunization data exemption form may be distributed in conjunction with the Vaccine Information Statements that are required to be disseminated by the National Childhood Vaccine Injury Act before giving any vaccine containing diphtheria, tetanus, pertussis, measles, mumps, rubella, polio, hepatitis A, hepatitis B, Haemophilus influenzae type b (Hib), influenza, pneumococcal conjugate, meningococcal, rotavirus, human papillomavirus (HPV), or varicella (chickenpox) vaccine. If anyone has actually been provided the form we would appreciate getting a copy as to date, none of the agencies who should have a copy have a clue what we are talking about when asked.  We have submitted a FOIA request to the IDPH for a copy and if we get it we will publish it.

    Why would a vaccinated person want to receive a form to opt out of a vaccination ‘that they already had”. Opting out of something you’ve already done, seems like a waste of time and paperwork.

      It is not a form to opt out of the vaccination, it is a form to opt out of the vaccine registry maintained by the State.

    Unfortunately, it’s like the Department to be remiss, and like the Governor to place blame on someone else. I’m going to follow-up with others on this issue.

    Please consider doing a detailed article on this issue even without a FOIA response. Given the stated purpose(s) of the law, the Executive office of the Illinois Governor would be careless to not exercise emergency powers to assure full cimpliamce of Governor’s administrative agents. I believe these agents include those who “don’t have a clue.”

    KA and JK – you didn’t make clear if you were talking about covid-19 treatments or the normal historical vaccines. Since the covid-19 gene therapy treatment is not really a “vaccine”, it doesn’t fall under this section of legislation, right? the list seems pretty clear on the actual vaccines that are required. Those “real” vaccines contain bits of actual virus, hence the language in the legs. the covid treatments do NOT contain any virus, so they wouldn’t fall under this language.

    If you asked about the normal set of vaccines that are available, then my apologies.
